Modafinil was first developed by the French pharmaceutical company Lafon Laboratories in 1976. Lafon was bought out by Cephalon Inc in 2001, and to this day Cephalon still makes the most popular name brand version of Modafinil, known as Provigil.
The discovery of Modafinil actually came about due to research on another related compound, Adrafinil. Adrafinil was first developed by Lafon in 1974. Modafinil and Adrafinil are very similar at a molecular level, with the only difference being that Modafinil lacks a hydroxyl group that Adrafinil has.
This extreme similarity is no coincidence, as the two compound are intrinsically linked. Testing to determine the mechanism of action of Adrafinil led to the discovery of Modafinil. It was found that in the body, Adrafinil is broken down in the liver into its component parts. The primary active component of Adrafinil was found to be Modafinil.
Adrafinil and Modafinil are classified as eugeroic compounds, meaning they promote wakefulness. While Modafinil has stimulant-like effects, there is strong evidence that suggests that Modafinil’s mechanism of action differs substantially from that of traditional amphetamine based stimulants.
The exact mechanism of action of Modafinil is still unknown. However, there has been research done that has shown Modafinil affects levels and transport of several neurotransmitters in the brain and central nervous system, including Dopamine, Histamine, Orexin, Epinephrine, and Norepinephrine.

Adrafinil, and later Modafinil, was first seen to cause alertness and increased locomotor activity in animal models. Because of this, the first tests done in human subjects was as a treatment for the sleep disorder narcolepsy.
The sleep disorder narcolepsy affects an individual’s ability to stay awake during the day. Regardless of sleep quality the night before, people with narcolepsy experience excessive daytime sleepiness and can often fall asleep multiple times per day without warning. Modafinil has been seen to improve wakefulness and reduce excessive daytime sleepiness in narcoleptics.
Multiple double-blind placebo-controlled clinical trials have shown that compared to a placebo, Modafinil is significantly better at improving scores on the Epworth Sleepiness Scale, reducing latency to sleep as measured by the Multiple Sleep Latency Test, and reducing the frequency and duration of sleep episodes as measured by the Maintenance of Wakefulness Test.
Modafinil was approved by the FDA as a treatment for narcolepsy in 1998. Since that time, it has also been approved to treat shift-work sleep disorder (sometimes just called shift work disorder), and as an adjunct treatment for the excessive sleepiness experienced with obstructive sleep apnea/hypopnea syndrome.
Shift-work sleep disorder (SWSD) is a circadian rhythm disorder. Circadian rhythm refers to the normal approximately 24-hour internal clock that humans have that causes use to be awake during the day and sleep at night. During to working late, overnight, or extended work shifts, some people must be awake during the night and sleep during the day. This can lead to SWSD, causing people to suffer from excessive sleepiness when awake, and insomnia when trying to sleep.

Popular Off-Label Uses of Modafinil
While the above sleep disorders are the only approved on-label uses of Modafinil, many people use the drug off-label for different purposes.
