As was to be expected from the previous research with Adrafinil, initial research studies using Modafinil in animals showed increased activity and wakefulness. Consequently, the first studies using Modafinil in humans were for improving the sleep disorder narcolepsy.
Narcolepsy is a sleep disorder that affects sleep and wakefulness. People with this condition experience persistent excessive daytime sleepiness that is not related to sleep deprivation. Additional symptoms may include falling asleep instantly and randomly throughout the day; cataplexy, which is brief periods of extreme muscle weakness and subsequent bodily collapse brought on by strong emotions; sleep paralysis, which is the sensation of being paralyzed when falling asleep or when waking; vivid hallucinations upon waking; and altered nighttime sleep patterns.
In multiple double-blind placebo-controlled clinical research studies, Modafinil has significantly increased wakefulness and reduced daytime sleepiness in patients with narcolepsy compared to a placebo. When comparing Modafinil to a placebo, scores on the Multiple Sleep Latency Test and the Maintenance of Wakefulness Test are both improved in patients who were given Modafinil.
Modafinil has also been studied in combination with other narcolepsy medications, specifically sodium oxybate. In one phase 3, randomized, placebo-controlled research study done on Modafinil and sodium oxybate both individually and in conjunction, the scientists determined that both of the active medications significantly improved scores on the Epsworth Sleepiness Scale, Maintenance of Wakefulness test, and the Clinical Global Impression of Change test compared to placebo. The researchers further concluded that the two medications in conjunction may be more effective than either medication alone.
Narcolepsy is the first sleep disorder that Modafinil was approved for. Since that time it has been approved for more sleep disorders, specifically shift-work sleep disorder and obstructive sleep apnea/hypopnea syndrome.
Shift-work sleep disorder is a circadian rhythm disorder that can occur when people need to shift their sleep schedules due to working late or overnight shifts. The people experience excessive sleepiness when they are awake working their shifts during non-standard awake hours (nighttime), and can experience insomnia when trying to sleep during non-standard sleep hours (daytime). This disorder is also seen sometimes in people who work excessively long shifts, such as some doctors and nurses.
In various double-blind placebo-controlled studies, Modafinil has been seen to improve wakefulness and reduce excessive sleepiness in people with shift-work sleep disorder significantly over placebo. In one study, people were given a dose of Modafinil or a placebo before a work shift; the people who took Modafinil had a reduction in the frequency and duration of attention lapses measured by the Psychomotor Vigilance test compared to placebo patients.
Modafinil has also been used as an adjunct therapy for people with sleep apnea/hypopnea. In patients with this condition, during sleep the muscles of the mouth relax, allowing the tongue and/or soft palate to move backwards and either partially or completely block the airway. This leads to either shallow labored breathing during partial blockage (hypopnea), or the cessation of breathing during total blockage (apnea).
Because sleep quality can be greatly diminished due to waking up multiple times during the night when breathing is impaired, many people with apnea or hypopnea experience excessive daytime sleepiness. Despite using the primary treatment of continuous positive airway pressure therapy to address the apnea/hypopnea, some patients still report having residual sleepiness. In a research study, it was found that a significantly higher percentage of patients who took Modafinil had normalized sleepiness ratings compared to placebo patients.

Common Off-Label Uses of Modafinil

Multiple double-blind research studies have tested using Modafinil to manage ADHD in adults. Some studies compared Modafinil to a placebo, while others compared both Modafinil and conventional medications to placebo.
One double-blind placebo-controlled three phase crossover study compared the effectiveness of Modafinil to both a conventional ADHD drug (dextroamphetamine) and to a placebo. It was found that compared to patients who received a placebo, patients who were given either of the two active medications had significantly betters scores on the DSM-IV ADHD Checklist. Due to its ability to produce results similar to a traditionally prescribed medication, the researchers suggested that Modafinil may be a viable alternative to conventional stimulants for the treatment of adults with ADHD.
Other studies have compared Modafinil to a placebo alone, and found that scores on various tests including the Attention-Deficit/Hyperactivity Disorder Rating Scale-IV School and Home Versions, the Clinical Global Impressions Improvement scale, and the Teacher and Parent ADHD Rating Scale-IV are all significantly improved using Modafinil over a placebo.
While Modafinil has still not been officially approved as a treatment for reducing fatigue in various conditions, and as a treatment for ADD/ADHD, there are many doctors who are informed of current research that will prescribe this drug for these purposes.
